Expense Reimbursement Forms

BC Expense Form

This form is for expenses incurred by Breastfeeding Counselors representing Breastfeeding USA in their communities. Expenses incurred for Committee or Workgroup activities must be reported on the Committee Expense Form found below.
Breastfeeding Counselors should send a BC Expense Form for any expense incurred on behalf of Breastfeeding USA, even if the BC does not want reimbursement (is donating the amount of the expense to the organization.) Expense reimbursement policies are on the form and will also be detailed in the Breastfeeding USA Policy Manual. Please contact finance@breastfeedingusa.org if you need assistance.

Committee Expense Form

This form is for expenses incurred by volunteers working in a Breastfeeding USA committee. Expenses incurred for BC or Chapter activities must be reported on the BC Expense Form found above. Please contact finance@breastfeedingusa.org if you need assistance.
